Shoulder pain is one of the most common musculoskeletal problems medical providers see and usually doesn’t spontaneously resolve without the right form of physical therapy (Spine 2005; The Rheumatologist 2006).

Not being involved in your care prevents me from providing specific treatment advice, but shoulder impingement syndrome is when the relatively small space between the top of your upper arm bone (humerus) and your shoulder blade (scapula) is too small to allow adequate space for tissue between them when moving your arm out to the side and/or above your head. There are multiple possible causes, which may include muscle weakness (Journal of Orthopaedic Sports Physical Therapy 2006), abnormal bone anatomy (Clinical Orthopaedics and Related Research 1983), tightness of muscles and/or ligaments (American Journal of Sports Medicine 1998) and/or posture (Manual Therapy 2005).

The key to overcoming shoulder impingement syndrome (as well as many other causes of shoulder pain) is a thorough physical examination to find out the specific cause(s) of impingement. For example, a weakened rotator cuff can allow your upper arm bone to move excessively upward during certain arm motions, thus compressing sensitive tissues and causing pain. There are four rotator cuff muscles, so it’s important to identify which of the four should be targeted so you don’t waste time doing exercises for muscles that aren’t impaired. Tightness of the chest muscles is also a common culprit and must be carefully screened for in the examination (Archives of Physical Medicine and Rehabilitation 2002).

When prescribed appropriately, physical therapy exercises and stretches have been shown to significantly reduce pain associated with shoulder impingement syndrome. However, a common mistake is prescribing too many exercises that can quickly sap anyone’s motivation to follow through with a program.

Prescribing exercises for any condition should always take safety and effectiveness into consideration. A great physical therapist will choose exercises that effectively strengthen multiple muscles simultaneously to reduce the time requirement. The only potential drawback is that these more effective exercises require more time to learn to do correctly. A great example is the prone arm press, an exceptional shoulder exercise that safely strengthens the rotator cuff, upper middle back muscles (rhomboids, middle and upper trapezius), back of the shoulders (posterior deltoid), neck (upper trapezius) and lower back (paraspinals).

Important things to note:

1. Very little weight is needed;

2. Tightness in your shoulders and chest will make the exercise more difficult;

3. Keep your head completely still, and;

4. Keep your head, chest, shoulders and arms close to the ground but don’t touch it.

Sets and repetitions should be established by your physical therapist.

With any rehabilitation, try to keep an open, honest dialogue with your physical therapist to help facilitate success. If you’re told to do a program that takes more time than you’re willing to commit to, it probably will be a waste of time.

Area residents resolve to get fit in 2012 – The Herald

At area gyms Monday, the busiest station was the sign-up desk as area residents made good on the popular New Year’s resolution to get in shape in 2012.

“We haven’t been this busy in a long time,” Planet Fitness employee Tahlullah Barger said, surveying the crowded South End Shopping Center fitness center Monday afternoon.

Barger counted 22 new members since the new year began.

Eric Piper, 27, of Williamsport, was unabashed about being part of the New Year’s gym membership surge.

“I’m just another sheep,” he said with a grin between sets on a rear deltoid flex weight-training machine at Planet Fitness.

Piper said his resolution was to get fit in the new year, and he set a goal to go to the gym five days a week.

“I’m fat, so it’s time to get in shape for the ladies,” he said.

Piper said his trick to sticking with his resolution was to have a workout buddy, his friend Tom Port, 21, of Hagerstown.

“This is my third attempt to go (to the gym) regularly,” Port said.

Judy Brewer, 60, of Hagerstown, and Ginny Messier, 63, of Falling Waters, W.Va., had the same idea.

“We’re going to help each other,” Brewer said of her new “gym sister.” Together, they plan to work out three times a week.

Lisa Shupp, 31, of Hagerstown, said her workout buddy will be her husband.

“I’m hoping he’s going to be able to motivate me, and I’ll be able to motivate him as well,” Shupp said.

Finding the time to work out is especially difficult for the couple because they have a 5-month-old child at home, she said.

“I’m really working hard to get back to my prebaby weight,” she said.

Shupp’s plan is to hit the gym every day for half an hour after work.

Heather Kurtz, 24, of Maugansville, said she has a deadline attached to her weight-loss resolution: her friend’s wedding this summer.

“It’s the beginning of the year. That gives me six months,” Kurtz said.

And her workout buddy? The bride-to-be.

The exercise should be repeated 10 times, then take a minute break, and repeat 10 more times. Reverse Fly

The reverse fly exercise is also wonderful for an athlete, if he or she is looking to increase strength in the rear deltoid muscles. You should sit down on the bench so that the feet are together, keeping your arms at your sides, while holding the dumbbells. The palms should be facing each other, then bend forward gradually, and let the arms hang down. The arms should be straight as you raise the dumbbells upward with the arms, and then stop as the arms are parallel to the floor. You want to hold this position for a few seconds, then go back to starting position, and repeat the exercise 10 times. This exercise is also great for an athlete because it can reduce the risk of injury to the shoulders, back, and arms, while also increasing overall flexibility.

Bent Over Lateral Raise

The bent over lateral raise should also be an exercise that an athlete uses to increase strength in the rear deltoids. This exercise can increase strength in the shoulders, add flexibility to the shoulders and arms, and also increase the strength in the back. For this exercise, you want to bend at the waist, place your feet shoulder length apart, and bend the knees slightly. Take the dumbbells in your hands and lift them upward, so that the dumbbells are right in front of you, making sure to keep the palms facing each other. The elbows should stay slightly bent during this exercise, in order to prevent injury, similar to the bend in the knees. You want to raise the dumbbells backward and up in a semicircle, then hold the position for a few seconds, before going back to starting position. This exercise should be repeated 10 times, but you should make sure to go back to starting position before starting another repetition. You also want to maintain the bend in the elbows, as well as the knees, so that the rear deltoid muscles are properly strengthened.

And while Mr. Gervais delivered by not holding back, it wasn’t an unpredictable or even a very seditious performance — a lot of the time his jokes felt forced and heavy-handed. Mostly, Mr. Gervais’s return as a naughty, nasty master of ceremonies — this was his third time — was a reminder that in show business, it’s all business.

Under all the froth, fuss and Champagne fizz, the Golden Globes are a sideshow that has swelled by straining and striving for industry relevance. Mr. Gervais put it less delicately. The Golden Globes are to the Oscars, he said, what Kim Kardashian is to Kate Middleton, “a bit louder, a bit trashier, a bit drunker and more easily bought.”

The Golden Globes are also a little like the Iowa caucuses: neither contest is a reliable harbinger, both are decided in a mystifying process by an unrepresentative group of voters, and yet merely by being first and so closely watched, both matter. So much so that eventual winners, like presidential candidates, almost always show up.

This year, it was Madonna, who beat out Mary J. Blige to win a Golden Globe for the song she co-wrote for her widely panned film about the Duchess of Windsor, “W.E.” Her victory was the quintessential Golden Globe trade-off — few will see the movie or hear Madonna’s song, but she was given the trophy because everybody wants to see her, or at least a glimpse of her bulging ego and biceps. She did not disappoint. She looked buff and delivered an acceptance speech that would have sounded a little too self-involved in a James Lipton interview on “Inside the Actors Studio.” A musical cue forced her to cut it short.

There were other idiosyncratic choices: Kate Winslet beat both Elizabeth McGovern (“Downton Abbey”) and Romola Garai (“The Hour”), and Kelsey Grammer won for “Boss,” even though he was up against Damian Lewis, the star of “Homeland,” which won the best television drama award. Laura Dern, the star of “Enlightened,” won against Amy Poehler, the star of “Parks and Recreation.”

There were flubs, including a wayward teleprompter that left Rob Lowe to exclaim, “This is why we love this evening.” At least one boorish remark didn’t even come from Mr. Gervais. Seth Rogen, presenting alongside a sexy Kate Beckinsale, told viewers he was trying to “conceal a massive erection.”

But beneath a scrim of informality, the Hollywood hierarchy remains intact. Mr. Gervais noted that movie stars were seated in the front, television stars on the sidelines. Aida Takla-O’Reilly, the president of the Hollywood Foreign Press Association told an interviewer on the red-carpet segment that the seating was “very complex and difficult,” like “the United Nations.”

Mr. Gervais popped back onstage throughout and his material kept getting drearier. He introduced “the Queen of Pop,” and then added, “Not you, Elton” to Elton John. Madonna, who holds the title, tried to pay Mr. Gervais back after he made a joke at her expense based on her old hit “Like a Virgin.”

“Why don’t you come over here and do something about it,” she said tauntingly. “I haven’t kissed a girl in a few years.” Madonna paused a beat then added, “On TV.”

As usual, George Clooney was appealingly relaxed and Morgan Freeman brought a touch of class with a charming speech after winning a lifetime achievement award. But it was Matt LeBlanc, formerly of “Friends,” who was the most disarming. He won a Golden Globe for playing himself on the comedy “Episodes” and thanked the writer-producers for writing, as he put it, “a Matt LeBlanc who, let’s be honest, is way more interesting and fun than the real thing.”

Carnival Wreck Timing May Exacerbate Loss in Cruise Business

Jan. 15 (Bloomberg) — The timing of Carnival Corp.’s Costa Concordia cruise ship’s grounding off the coast of Italy, at the start of the peak booking season, may exacerbate the company’s losses.

About one-third of all cruise vacations are arranged during the so-called wave season from January to March, said Sharon Zackfia, an analyst with William Blair Co. in Chicago.

“They’re the most profitable bookings,” Zackfia said in an interview. “Presumably most people now are booking for the key summer season, which is when the cruise lines make the bulk of their money.”

Europe accounted for about 38 percent of Carnival’s revenue in fiscal 2010, the last full year for which geographic results are available. Its Genoa-based Costa Crociere unit is the continent’s largest cruise line based on passengers and ship capacity, according to Miami-based Carnival, the world’s biggest cruise operator with brands that include Cunard and Princess Cruises.

Carnival will lose business from customers who were booked on future Concordia voyages, Zackfia said. There will be additional costs that are hard to estimate, she said. The company doesn’t have insurance that covers lost revenue or earnings from its ships or other operations, according to its most recent 10-K, filed in January 2011.

“They will obviously have customers who were booked on upcoming Concordia journeys that aren’t going to happen,” Zackfia said.

Death Toll

Five people were confirmed dead and about 15 remain missing from the Costa Concordia, which ran aground the night of Jan. 13 near the island of Giglio in the Tyrrhenian Sea. Three people were found alive in the capsized cruise liner. The ship’s captain has been arrested and accused of manslaughter, abandoning the vessel and causing the shipwreck.

Rescue workers evacuated more than 4,000 of the 4,229 passengers and crew aboard, Italy’s Civil Protection agency said on its website. Costa passengers come primarily from Italy, France and Germany, according to Carnival.

Search and rescue operations will continue through the night, according to a statement today by the regional government in the province of Grosseto. Costa Crociere has been ordered to remove the ship, according to the statement.

“I want to express our deep sorrow for this terrible tragedy,” Gianni Onorato, president of Costa Crociere, said in a statement. “I am only now able to speak on behalf of Costa because, as you will understand, I have been at Isola del Giglio to be close to the rescue operations.”

Ship Itinerary

The ship had embarked from Rome on a trip that was to include stops at ports in France and Spain. The liner hit rocks and Captain Francesco Schettino, after assessing the damage, decided to secure the ship and gave the evacuation order, Onorato said.

Costa has 15 ships and sails globally. The company agreed in 2007 to pay 510 million euros ($647 million) each for two Concordia-class ships. The Costa Favulosa, added to the fleet last year, and the Costa Fascinosa, scheduled to enter service in May, was to increase Costa’s capacity by 20.4 percent, according to company filings.

Carnival carries insurance that covers a number of risks within certain limitations, according to the January 2011 filing. The coverage includes hull and machine insurance, as well as protection and indemnity policy that includes crew and passenger injuries, shipwrecks, damage to third parties and pollution.

Insurance Coverage

The company also had practiced some self insurance in the past, according to Zackfia and to filings. Carnival had no immediate response to requests for specifics of its coverage.

The Costa Concordia was insured by companies including Assicurazioni Generali SpA, RSA Insurance Group Plc and XL Group Plc, said three people with knowledge of the policies. The three are among several insurers facing total costs of about 405 million euros ($512 million), said one of the people, who declined to be identified because policy terms are confidential.

“This is a company that has a very strong, solid cash flow and balance sheet,” Zackfia said.

With U.S. markets closed tomorrow for the Martin Luther King Jr. holiday, Carnival will trade for the first time following the incident in London. Carnival fell 2.5 percent in New York on Jan. 13, valuing the company at $27.8 billion. The shares declined 29 percent last year.

Bookings had been “strong” heading into the wave season, Chairman and Chief Executive Officer Micky Arison said in a Dec. 20 statement, with “slightly higher prices with slightly lower occupancies.”

Company Earnings

For the year ended in November, revenue rose 9.2 percent to $15.8 billion, Carnival said on Dec. 20. The company projected then that net income would rise to $2.55 to $2.85 a share this year, adjusted for one-time items, from $2.42 in fiscal 2011.

The company’s last major incident happened in November 2010, when an engine fire aboard Carnival Splendor stranded the cruise ship off the California and Mexican coasts for days, with more than 4,400 passengers aboard. Splendor, which has since been repaired, was used for Mexican Riviera voyages from Long Beach, California.

“Historically, they’ve had a very good safety track record,” Zackfia said. “This is definitely a tragic event. I’m sure the company is taking this seriously.”

Dallas, which just swept four games in five nights, isn’t just winning. The Mavs are winning big, posting margins of victory of 15, 14, five, 26 and 39 points during this streak.

Granted, it’s probably not wise to get too carried away given the quality of competition. The Mavs just whipped up on a bunch of teams with losing records over the last week and change.

“We played the way we were supposed to,” sixth man Jason Terry said after scoring a game-high 21 points while the Mavs set franchise records for fewest points and lowest field-goal percentage (.256) allowed in a game.

The defending NBA champions get a great chance to prove this run hasn’t been fool’s gold during a four-game road trip that begins with a stop in L.A. to play a pair of Western Conference contenders, the Lakers and Clippers.

“If we compete and play with energy on both ends of the floor,” Nowitzki said, “I like our chances against anybody.”

The Mavs clearly weren’t prepared to face quality competition when the season started on Christmas. They were embarrassed by being blown out at home by the Miami Heat and Denver Nuggets, but this is a totally different team just a few weeks later.

The “old, slow and out of shape” Mavs, as superstar Dirk Nowitzki labeled them after a couple of games, have at least gotten in shape. This cast will never be young or quick, but they’re back to playing the kind of defense that keyed last season’s title run.

Dallas hasn’t allowed an opponent to score 100 points in the last 10 games. The Mavs are 8-2 in that stretch and haven’t allowed a 90-point outing in any of their victories.

If the Mavs keep locking it down like that, the offense will flow just fine in time.

“Defensively is where it’s at,” Nowitzki said after a ho-hum offensive performance, scoring 14 points on 6-of-14 shooting. “Over the last couple of games, we really established that first. That’s where we were at our best last year, when we got stops and didn’t get stagnant offensively.”

Cuban made the point before the game that all of the Mavs’ veterans will have what appear to be down seasons because the plan is for them all to play fewer minutes with the condensed schedule. The Mavs routinely have at least 10 or 11 men play significant minutes each game.

The bench often outscores the starting lineup, as was the case Saturday night, when the second unit led by Terry and Vince Carter (16 points, 6-8 FG) had 68 points to also outscore all of the Kings. The Mavs are buying into Carlisle’s demands for maximum effort with a focus on doing the dirty work.

“The three major points we’re stressing are energy, competiveness, defense and rebounding,” Mavs coach Rick Carlisle said. “So that’s four, but actually the first two are one. Work with me on that.”

It’s working really well for the Mavs right now.
